St. Edward Catholic School

Each school sport requires 2 eligibility documents:

1) A physician signed Archdiocesan Sport Physical (“Pre-Participation Physical Evaluation 2024-2025 School Year” Form) 
This is located on the website and expires a year after the physician signature.  If your child plans on participating in the athletic program, it is recommended that they receive a sports physical in the summer.  This would allow the document to last them the entire school year.  Each year the diocese updates the forms and the form for that current school year, for example 2024-2025 is required.  The Pre-Participation Physical Evaluation 2024-2025 Form will be posted this summer online for your convenience.

2) SECS Athletic Registration and Parental Consent Form
This is a St. Edward Catholic School document requiring signatures for your child to participate in the athletic program.
